Welcome to PH Connect
PH Connect is an online tool designed to support Australian healthcare professionals (HCPs) in referring suspected Pulmonary Arterial Hypertension (PAH) patients to PAH expert centres for diagnosis and treatment. The tool provides you with guidance on raising the suspicion of PAH and assists in the development of a complete referral appropriate for sending your patient to a PAH expert centre. PH Connect is only recommended for use by physicians familiar with diagnostic methods such as transthoracic echocardiography.
Complete a short questionnaire to receive immediate guidance on developing a referral for your patient. The questionnaire is comprised of a set of questions based on criteria that are helpful in raising the suspicion of PAH. The questions have been adapted from the 2015 ESC/ERS Guidelines for the diagnosis and treatment of pulmonary hypertension, as well as updates from the 2018 World Symposium on Pulmonary Hypertension. It is not compulsory to complete all of the questions; however, providing more information will improve the quality of the guidance and, if necessary, the information in the patient’s referral.
Once submitted, the questionnaire will generate one of three potential conclusions:
- Consider additional testing – This summary will outline additional core tests that should be considered prior to referral. The summary will also provide information pertaining to these tests.
- This summary will advise you to use the PAH expert centre finder where you can find the most appropriate PAH expert centre for your patient and details for immediate referral.
- Complete PAH screening for Scleroderma patients – This summary will recommend you complete annual PAH screening of scleroderma patients, who are not presenting with PAH symptoms
The summary is generated by passing the questionnaire through a logic test, which will determine if any of the inputs should trigger a conclusion. The logic test is based on approved ESC/ERS Guidelines and has been corroborated with a panel of PAH experts.
Due to the haemodynamic definitions of PAH, it is only possible to make a definitive diagnosis in a PAH expert centre using right heart catheterisation. PH Connect is not a diagnostic tool and does not replace expert clinical judgement or necessary procedures. It is only intended to support the referral process for suspected PAH patients.
Choosing an incorrect option for any of the questions due to misunderstanding can lead to the generation of the wrong summary at the end of the questionnaire. To avoid misunderstanding, there is extra information provided for each question, which can be accessed by clicking on the “i” icon. In some cases, there will be further information provided via links to other websites.
